Establishment of Fixed Price Contract for Procurement of Medical Consumables Dialysis

OPTION CLAUSE: The Purchaser reserves the right to increase or decrease the quantity to be ordered up to 25 percent of bid quantity at the time of placement of contract. The purchaser also reserves the right to increase the ordered quantity up to 25% of the contracted quantity during the currency of the contract at the contracted rates. The delivery period of quantity shall commence from the last date of original delivery order and in cases where option clause is exercised during the extended delivery period the additional time shall commence from the last date of extended delivery period. The additional delivery time shall be (Increased quantity ÷ Original quantity) × Original delivery period (in days), subject to minimum of 30 days. If the original delivery period is less than 30 days, the additional time equals the original delivery period. The Purchaser may extend this calculated delivery duration up to the original delivery period while exercising the option clause. Bidders must comply with these terms.

• 1.Past Performance Criteria (Mandatory):
• a)Past Performance: The bidder must have successfully executed/completed contr acts of same or similar categories of the items to any Central/State Govt. Hospitals/ PSUs/NABH Accredited Hospitals in India. Bidder shall submit at least 03 order copies of same or similar items supplied durin g the period from April 2023 up to the bid end date. Note:
• 1.Non-submission of documentary proof w.r.t past performance crit eria shall result in outright rejection.
• 2.Bidder shall submit documentary proof w.r.t past performance criteria Co ntract/Order Copies/CRACs with self-certification by the bidder and/or e ndorsed satisfactory performance certificate (s) issued from respective co mpetent authority of the organization. "Similar Items" shall mean supply of Medical consumables any other related items t o any Central /State Govt Organization/PSUs/NABH Accredited Hospitals in India.
• 2.Registration with Regulatory Authorities (Mandatory)
• a)Self-Declaration w.r.t Applicable Licenses: The Service Providers shall submit a self-declaration of all applicable licenses and certifications e.g., validity and authe nticity/genuineness of drug license, product certification, manufacturer certification /licenses, test reports etc. The Service Provide shall ensure that all mandatory licen ses shall be valid during the contract period
• b)Licence issued by Drug Controller Authority: The Service Provider shall submi t a valid license issued by Drug Controller Authority of the State for running the che mist shop/establishment, and Wholesale License issued by DCGI/ SDC or Import Lic ense in case the product comes under Drugs and Cosmetics Act, 1940. The success ful Service Provider will further ensure that their Licenses remain valid till the end o f the contract period. All Provision of Drugs & Cosmetic Act 1940 as amended till da te and rules made there under will always be applicable on Service Provider. The Service Provider shall submit a Non-conviction Certificate from the State Dr ug Controller and must be complied by the Service Provider as specified by Buyer. The Non-Conviction Certificate should be dated not more than 6 months prior to dat e of publishing of Bid.
• 3.This tender is for long time supply of Medical Consumables. Hence, the offered prices shall be valid for a period of 02 years. The buyer commits to procure minimum of 30% of contract value during the currency of the contract and the tender val ue is tentative.
• 4.Sample(s): Sample of the quoted Medical Consumables shall be submitted by the bidd ers, if required.
• 5.In addition to the facility of depositing EMD/Bid Security on an item-wise or schedule- wise basis on the GeM portal, bidders participating for multiple items or schedules ma y also deposit a consolidated EMD/Bid Security.
• 6.Payment Terms: Payment shall be made within 30 days from the date of CRAC subject to submission of invoices or required documents as per the terms & conditions of the contract/tender. .
