Bidder shall submit the following documents along with their bid for Vendor Code Creation:
Data Sheet of the product(s) offered in the bid, are to be uploaded along with the bid documents. Buyers can match and verify the Data Sheet with the product specifications offered. In case of any unexplained mismatch of technical parameters, the bid is liable for rejection.
Manufacturer Authorization:Wherever Authorised Distributors/service providers are submitting the bid, Authorisation Form /Certificate with OEM/Original Service Provider details such as name, designation, address, e-mail Id and Phone No. required to be furnished along with the bid
OPTION CLAUSE: The Purchaser reserves the right to increase or decrease the quantity to be ordered up to 50 percent of bid quantity at the time of placement of contract. The purchaser also reserves the right to increase the ordered quantity up to 50% of the contracted quantity during the currency of the contract at the contracted rates. The delivery period of quantity shall commence from the last date of original delivery order and in cases where option clause is exercised during the extended delivery period the additional time shall commence from the last date of extended delivery period. The additional delivery time shall be (Increased quantity ÷ Original quantity) × Original delivery period (in days), subject to minimum of 30 days. If the original delivery period is less than 30 days, the additional time equals the original delivery period. The Purchaser may extend this calculated delivery duration up to the original delivery period while exercising the option clause. Bidders must comply with these terms.
Products supplied shall be nontoxic and harmless to health. In the case of toxic materials, Material Safety Data Sheet may be furnished along with the material.
Shelf Life: The Product/Spare parts to be supplied as part of the services must have minimum Two years or more as per DCGI and CDSCO Shelf Life. On the date of supply, minimum Five Sixth remaining as on Date of Supply usable shelf life should be available / balance.
While generating invoice in GeM portal, the seller must upload scanned copy of GST invoice and the screenshot of GST portal confirming payment of GST.
Nominated Inspection Agency: On behalf of the Buyer organization, any one of the following Inspection Agency would be conducting inspection of stores before acceptance: Pre-dispatch Inspection at Seller Premises (applicable only if pre-dispatch inspection clause has been selected in ATC): Nil Post Receipt Inspection at consignee site before acceptance of stores: Medical officer incharge medical stores and Head of Department
Testing of Sample: The testing of advance sample and bulk sample during PDI will be carried at the designated AHSP labs.When testing facilities are not available, the facilities of Govt labs/NABL/Accredited labs will be utilized. The testing charges outside the designated AHSP labs to be borne by seller's.
Bidder's offer is liable to be rejected if they don't upload any of the certificates / documents sought in the Bid document, ATC and Corrigendum if any.
ISO 9001: The bidder or the OEM of the offered products must have ISO 9001 certification.
The bidder is required to upload, along with the bid, all relevant certificates such as BIS licence, type test certificate, approval certificates and other certificates as prescribed in the Product Specification given in the bid document.
To be eligible for award of contract, Bidder / OEM must possess following Certificates / Test Reports on the date of bid opening (to be uploaded with bid): Mandatory Certification and Licence as mentioned in the bidding documents and as per Drug Controller Ge neral of India and Central Drug Standard Control Organisation and National Pharmaceutical Pricing Authorit y and Medical Device Rules 2017 as amended till date. .
Buyer Added text based ATC clauses Shelf life : Two years or more as per Drug Controller General of India and Central Drug Standard Control Organisation Five sixth shelf life remaining as on the date of supply of medicines / invoice. Shelf life of each quoted drugs/medicines shall be in accordance with Schedule P of Drugs and Cosmetics Rules, 19 45 as amended up to date. Undertaking to replace the medicines six months before the expiry period if the stock is unconsu med Recalls. If products are recalled because of problems with product quality or adverse reaction to t he pharmaceutical, the supplier will be obliged to notify the buyer, providing full details about the r eason leading to the recall, and shall take steps to replace the product in question at suppliers ow n cost at the ultimate destination with a fresh batch of acceptable pharmaceuticals or withdraw an d give a full refund of the unconsumed quantity if the product has been taken off the market due t o safety problems. Cold Chain Maintenance as per requirement of medicines and Drug Controller General of India Packaging, labelling and marking requirements as per Drug Controller General of India and Central Drug Standard Control Organisation Bar Coding. All drugs/medicines supplied should incorporate GS1 barcodes standards at various packaging levels (primary, secondary, and tertiary level packaging) and should encode the inform ation within the barcodes as mentioned by the buyers in addition to other existing statutory labelli ng and marking requirements Mandatory Drug Licence. as mentioned in the bidding documents and as per Drug Controller G eneral of India and Central Drug Standard Control Organisation and National Pharmaceutical Pricin g Authority. Drug Manufacturing Sales Distribution License Number / Manufacturing Unit Certificati on. Drug Manufacturing License date / Valid Medical Device Licence (CDSCO) for the product i ssued from the Competent Authority defined under Drugs And Cosmetic Act 1940 and Rules made there under as amended till date including Quality test by Statutory Authorities. Compliance to Medical Device Rule (MDR) 2017 as amended till date Availability of Test Report / NABL Test report for the batch for each supplied batch/ products as per DCGI, CDSCO and Medical Device Rule (MDR) 2017 as amended till date. If any batch of an y product(s) supplied by the supplier is declared “NOT OF STANDARD QUALITY”, by any governme nt agencies or drug licensing authority, the supplier shall inform the same immediately to the buye r so that the use of the available stock of the particular item will be stopped. Further, the available stock of the product will be returned and the amount as per contract will have to be submitted by the firm/distributor/vendor or else will be recovered. In case of failure of the test or supply of subst andard stores, further actions will be initiated as per DPM 2025 and the firm will be banned/suspended. Submission of all necessary certifications, licences and test reports as per requirement including GMP, GLP and undertaking regarding non conviction/no blacklisting of OEM, Distributor, Owner, dir ector, proprietor, partner from Central / State Govt due to non compliance. Compliance to the QRs/Specifications attached. The price offered by the seller/bidder shall not, i n any case, exceed the DPCO/NPPA controlled price or price fixed by the State Government, if any. The seller must reduce the prices if there is any reduction in DPCO/NPPA ceiling price or price fixed by State Government, if any. Maximum Retail Price and Brand Name of the subject medicine/cons umable to be mentioned in compliance document Latest Supply orders of the subject medicine/consumable to other Armed Forces Hospitals or other reputed institutions / hospitals All terms and conditions of Defence Procurement Manual 2025, Manual of Procurement of Goods 2 024, Orders and instruction of GEM Manual, CVC Guidelines and Medical Schedule of Powers 2022 are applicable for this procurement.
