• A)Past work experience: Bidders should submit the successfully executed/completed work order for NDT jobs in c ontinuous process industry like fertilizer/ petrochemicals/refineries/power plants/steel/ oi l and gas industry, The work order shall be issued & executed during the last seven year s ending last day of month previous to the one in which applications are invited and fulfill ing either of following: Minimum value of completed work must be as follows: Three similar completed works each costing not less than the amount equal to Rs.9.93 La khs (excl. GST). OR Two similar completed works each costing not less than the amount equal to Rs.12.41 La khs (excl. GST). OR One similar completed work each costing not less than the amount equal to Rs.19.86 Lak hs (excl. GST). Proof of execution of submitted work order, vendor shall submit any of the following –
• (i)Completion Certificate / Customer Acceptance Certificate OR
• (ii)GeM SDAC (Service Delivery Acceptance Certificate) document OR
• (iii)(a) Tax Invoice to the extent of the required Pre-Qualification value and
• (b)Proof of receipt of payment like bank statement for the above referred tax invoice
• (iv)The document submitted as proof of execution must be specifically for the same concer ned Purchase Order (PO)/Work Order/Contract submitted, clearly indicating the correspondi ng PO/Work Order/Contract number. Note: The reference work orders produced by the tenderer must be placed on them directly by th e owner organization. The job carried out by contractor for other agency as a sub- contract or will not be considered.
• B)Average annual financial turnover during the last three years, ending 31st March of the previous financial year (i.e. FY 2022-23, 2023-24 & 2024-25should be at least Rs.7.45 Lak hs. In case the date of constitution/incorporation of the bidder is less than three years old, the average annual financial turnover in respect of the completed financial years after the date of constitution shall be taken into account. In this case, the financial turnover during last fin ancial year/ the average financial turnover during preceding two financial years should be at least Rs.7.45 Lakhs. Bidders shall submit CA certificate / CA audited Profit & Loss accounts, Balance sheets etc. i ndicating the annual turnover figures, in support of the same. The financial documents has to be CA audited/issued with membership number, seal, signed and with UDIN.
• C)Ownership of company with name, addresses and telephone nos. of proprietors/ partner s/directors.
• D)Copy of Partnership deed /proprietorship/ Memorandum and article of association and c ertificate of incorporation as the case may be.
• E)All the bidders have to submit the Net worth certificate of last three financial years (FY 2 022-23, 2023-24 & 2024-25) issued by CA with membership number, seal, signed and with UDIN or CA audited financial statements/balance sheets for each of the last three fin ancial years. The net worth should not be negative in the balance sheet of last financial year (i.e. net wo rth should not be negative on ‘The Relevant Date’). AND The Net worth of Bidder should have not eroded by more than 30% in the last three years, ending on ‘The Relevant Date’. It means the net worth of last financial year should not be l ess than 70% compared to the 3rd last financial year. In case, the date of constitution/ incorporation of the bidder is less than 3 years old, the foll owing will be applicable – Ø The net worth in each of these years should not be negative Ø Net worth in respect of the immediate previous financial year should not have eroded more than 30 percent than that of its previously audited financial statement. In case the bidder has completed financial statements of only preceding year, the clause of n et worth erosion will not be applicable. “The Relevant Date” means 31-03-2025 for block of FY 2022-23, 2023-24 & 2024-25.

• 1.PART-II: PRICED BID:- For tender on GeM portal – Follow GeM guidelines as mentioned in GeM portal. Also submit the price break-up in the format as given with the tender. Bidder have to quote for all items mentioned in the price break-up f ormat. The quoted item wise unit rates must preferably be in whole numbers only, pls. avoid to quote item wise unit rates in decimal pl aces. Vendor should quote prices as per above only. Offers indicating rates a nywhere else (scanned documents etc.) shall be liable for rejection. .
