• i)Valid Manufacturing Licence of manufacturer/OEM. ii) Certificate of Good Manufacturing Practice (GMP): 1) under Schedule M/revised Schedule M of Drug & Cosmetic Act/ WHO GMP and if license is issued by state Food licensing authority t hen ISO certificate/Quality Management system (QMS) and 2) If license issued under Medical Devices rules (MD-5), then no re quirement of QMS/GMP/ISO from Principal Manufacturer. iii) Complete Specification of the quoted item (including label of produc t). iv) Certificate that the rates quoted by them are not higher than those being quoted with other Government, Public Sector or Private Organizat ions.
