Provision of Automatic Fire Detection and Alarm System(AFDAS) at Independent Interlocked LC Gates over JP Division.

The tenderers shall submit a copy of certificate stating that all their statements/documents submitted alongwith bid are true and factual. Standard format of certificate to be submitted by the bidder is enclosed as Annexure-V

In addition to Annexure-V, in case of other than Company/Proprietary firm. Annexure -V(A) shall also be submitted by the each member of a Partnership Firm / Joint Venture (JV) / Hindu Undivided Family (HUH / Limited Liability Partnership (LLP) etc, as tlre case may be. Non submission of above certificate(s) by the bidder shall result in summarily rejection of their bid.

List of plants & Machinery available on hand (own) and proposed to be inducted (own and hired to be given separately) for the subject work in Annexure-VI.

Tenderer must submit details of Annual Contractual Turnover Data for the Previous 3/4 Years (Contractual Payment only) etc. for fulfilling financial criteria as per GCC 2022 and latest amendments to it in Annexure VI(B).

List of Personnel, Organization available on hand and proposed to be engaged for the subject work in Annexure -VII.

Tenderer should submit list of deviations in offer (if any) from Tender conditions only in prescribed proforma vide Annexure VIII.

For Sole Proprietorship firm (a) Affidavit as per proforma given of Annexure IX (duly executed on stamp paper and notarized).

In case one or more members of JV is/are Proprietary Firm or HUF (Hindu Undivided Family) the following documents shall be Uploaded/Submitted (a) A copy of notarized affidavit on Stamp Paper declaring that he who is signing the affidavit on behalf of HUF is in the position of 'Karta' of Hindu Undivided Family (HUF) and he has the authority, power and consent given by other members to act on behalf of HUF (Annexure-IX(A)).

For JV firm b) Memorandum of Understanding of JV as per pro forma given in Annex. X (duly executed on stamp paper and notarized).

For JV firm c) Power of Attorney/authorization duly Notarised by all JV constituents, in favour of the individual under whose digital signature key the tender document shall be uploaded on behalf of JV for signing the tender document on behalf of the JV (Standard Performa as per Annexure-X(A)

In case one or more of the members of the JV is/are partnership firm(s), following documents shall be submitted: (a) A notorized copy of Partnership Deed(notarized prior to date of tender opening as per the Indian Partnership Act.).

In case one or more of the members of the JV is/are partnership firm(s), following documents shall be submitted: (b) A copy of consent of all the Partners or individual authorized by partnership firm to enter into JV Agreement as per pro forma given in Annexure-XI(duly executed on stamp paper).

In case one or more members of JV is/are Proprietary Firm or HUF (Hindu Undivided Family) the following documents shall be Uploaded/Submitted: (b) A copy of Special Power of Attorney to be submitted by Sole Proprietor or HUF participating as member of JV firm as per proforma given in AnnexureXII (duly registered with the Registrar or notarized).

In case one or more members of the JV is/are companies, the following documents shall be submitted: d) A copy of Authorization/copy of Power of Attorney issued by the Company (backed by the resolution of Board of Directors) in favour of the individual to sign the tender, , sign MOU/JV Agreement on behalf of the company and create liability against the company, as per proforma given in Annexure-XII(duly registered with the Registrar or notarized).

For Partnership firm (b) Special Power of attorney to be submitted by Partnership firm in favour of the individual to sign the tender on behalf of the firm and create liability against the firm as per proforma given in Annexure-XIII (duly registered with the Registrar or notarized).

For Partnership firm (a) A copy of Partnership Deed (notarized prior to date of tender opening as per the Indian Partnership Act.)

For Sole Proprietorship firm b) Special power of Attorney to be submitted by sole proprietor firm as per proforma given in Annexure XIV(duly registered with the Registrar or notarized). (Not required if tender documents are submitted by proprietor himself as per (a) above)

For Company registered under Companies Act 2013: (a)The copies of MOA (Memorandum of Association) / AOA (Articles of Association) of the company.

For Company registered under Companies Act 2013: (b) A copy of Certificate of Incorporation

For Company registered under Companies Act 2013: (c)A copy of Authorization/Power of Attorney issued by the Company (backed by the resolution of Board of Directors) in favour of the individual to sign the tender on behalf of the company and create liability against the company.as per proforma given in Annexure- XV&XV(A)(duly registered with the Registrar or notarized).

In case one or more members of the JV is/are companies, the following documents shall be submitted: a) A Copy of Memorandum & Articles of Association of Company.

In case one or more members of the JV is/are companies, the following documents shall be submitted: b) A Copy of resolution passed by Board of Directors of the company, permitting the company to enter into a JV agreement to be submitted as per Annexure-XVII.

In case one or more members of the JV is/are companies, the following documents shall be submitted: c) A copy of Certificate of Incorporation.

In case one or more of the members of the JV is/are partnership firm(s), following documents shall be submitted: (c) Special Power of attorney to be submitted by Partnership firm in favour of the individual to sign the MOU/JV agreement on behalf of the Partnership Firm and to create liability against the firm as per proforma given in Annexure-XVIII (duly registered with the Registrar or notarized).

Contractual payment received in last three years -in support of Bid Capacity(Mandatorily for tender value more than Rs.20 crore) Annexure- XIX if contractual payments received are based on Audited Balance Sheets duly certified by Chartered Accountant. OR Annexure- XIX(A), if contractual payments received from concerned Department / Client based on Form 26AS/16A from IT TRACES duly certified by Chartered Accountant.

For LLP (Limited Liability Partnership) Firm Registered under LLP Act 2008: d) Special Power of Attorney/ Authorization issued by LLP firm in favour of the individual to sign the tender on behalf of the LLP firm and create liabilities against the LPas per proforma given in Annexure-XX(duly registered with the Registrar or notarized).

For LLP (Limited Liability Partnership) Firm Registered under LLP Act 2008: (a)A copy of LLP Agreement.

For LLP (Limited Liability Partnership) Firm Registered under LLP Act 2008: (b)A copy of certificate of Incorporation

For LLP (Limited Liability Partnership) Firm Registered under LLP Act 2008: c) A copy of resolution passed by partner of LLP firm for submitting tender by LLP firm and to deal with tender on behalf of the firm as per proforma given in Annexure-XXI.

For Registered Society & Registered Trust (a) A copy of the certificate of registration.

For Registered Society & Registered Trust (b) A copy of Deed of Formation.

For Registered Society & Registered Trust (c) A copy of Special Power of Attorney in favour of the individual to sign the tender documents and create liabilities against the Registered Society/ Trust as per proforma given in Annexure-XXII (duly registered with the Registrar or notarized).

In case one or more members of JV is/are LLP firm, the following documents shall be submitted/uploaded: (a) A notarised copy of certificate of incorporation and LLP agreement.

In case one or more members of JV is/are LLP firm, the following documents shall be submitted/uploaded: (b) A notarised copy of resolution of the partners of LLP, permitting the LLP to enter into a JV agreement, authorising one of the partners of LLP to sign JV MOU/agreement and such other documents required to be signed on behalf of the LLP and to create liability against the LLP and/or to do any other act on behalf of LLP (Standard proforma as per Annexure-XXIV)

In case one or more members of JV is/are LLP firm, the following documents shall be submitted/uploaded: (c) A copy of authorisation/copy of power of attorney issued by the LLP (backed by resolution of partners) in favour of individual to sign the tender, sign MOU/JV agreement on behalf of the LLP and create liability against the LLP (Standard proforma as per Annexure XXV)

For Partnership firm (C)Document(s) in support of Registration of firm with Registrar of firms which includes Certificate of registration and Register of firm (Form No. may vary from State to State) (as applicable) etc. issued by Registrar of firms.

For partnership firm (d)If tender is submitted by Existing or newly formed Partnership Firm, relevant Annexure XXVI and XXVII as declaration to be submitted by the tenderer along with tender document.

For JV firm (a) All the Members of JV shall certify that they are not blacklisted or debarred by Railways or any other Ministry / Department of the Govt. of India from participation in tenders/contract on the date of opening of bids either in their individual capacity or as a member of the JV in which they were/are members.

For HUF (a) A copy of notarized affidavit on Stamp Paper declaring that he who is submitting the tender on behalf of HUF is in the position of 'Karta' oHindu Undivided Family (HUF) and he has the authority, power and consengiven by other members to act on behalf of HUF. (Standard proforma as pe Annexure XXVIII).

A Power of Attorney executed and issued overseas, the document will also have to be legalised by Indian Embassy and notarized in the Jurisdiction where power of attorney is being issued. However, the POA provided by the bidders of the country who have signed the Hague legislation convention 1961 are not required to be legalised by the Indian Embassy if it carries a conforming Appostille certificate. (As per correction slip in GCC 2022.

The tenderer shall submit the self attested copy of the PERMANENT ACCOUNT NO. issued by the Income Tax Department.
