• 4.Income Tax Return acknowledgement of the firm for last 3 years.

• 8.Eligibility Criteria for Bidders: The bidder must be one of the following: · The manufacturer, · A 100% subsidiary of a foreign manufacturer operating in India, · The sole authorized distributor of the manufacturer, or · A direct authorized distributor of the manufacturer. Note: Sub-authorization (i.e., authorization granted by an intermediary rather than the manufacturer) will not be accepted.
• 9.Certification regarding Make in India (MII) as per OM dated P-45021/2/2017-PP (B.E-II) dated 15/06/2017, as amended from time to time and as applicable on the date submission of tender. (The declaration from your OEM manufacturer specifying the exact percentage of local content alongside the de tailed address of the location where the local value addition is performed, to be enclosed with the bid.)
• 10.Notary Affidavit on non-Judicial Stamp paper of Rs. 100.00 and on Letter Head also. (Format enclosed- Affidavit)
• 11.Disclosure of existing customers to whom supply of equipment / service made available in India.
• 12.GeM Tender Document (Bid) should be duly signed & stamped by vendor.
• 13.Compliance required against O.M. F.7/10/2021-PPD dated 23/02/2023 issued by Ministry of Finance w.r .t. restrictions under Rule 144 (xi) of General Financial Rules (Format Enclosed at Page No. 16 and 17. • 1.The contractor shall maintain the CCTV system in safe, reliable and operati onal condition throughout the contract period.
• 2.All maintenance activities shall be carried out by competent and trained pe rsonnel.
• 3.The contractor shall arrange all tools, tackles, testing instruments, safety e quipment and transportation required for the work.
• 4.The contractor shall follow ACTREC's safety, security and work permit proc edures.
• 5.The contractor shall immediately inform the Engineer-in-Charge regarding any major defect, abnormality or condition requiring replacement outside t he AMC scope.
• 6.No major component excluded from the scope shall be replaced without pr ior written approval from ACTREC.
• 7.The contractor shall maintain records of all preventive and breakdown mai ntenance activities.
• 8.The contractor shall ensure minimum disruption to critical hospital activitie s while carrying out maintenance.
• 9.The contractor shall comply with applicable GeM General Terms and Condit ions and TMC procurement rules.
• 10.Technical evaluation shall be carried out based on compliance with the elig ibility criteria and technical scope. Financial bids of technically qualified bidders shall be considered for further evaluation, with award to the lowest responsive bidder in accordance with applicable procurement provisions.
• 11.Within 7 days from the commencement of the contract, the successful bidder shall inspect C CTV system submit an Initial Condition Assessment Report to ACTREC, covering the condition and operational status of all major equipment and components.
• 12.Any major pre-existing defect/fault identified during the initial inspection shall be clearly recor ded in the Initial Condition Assessment Report and brought to the notice of ACTREC. Such pre-exist ing defects shall not be considered part of the routine AMC scope. Repair/replacement of su ch defects, if required, shall be carried out either by ACTREC or by the AMC agency only after ob taining prior approval from ACTREC and at an additional cost, as applicable.
• 13.The contract may be extended for a further period of one (1) year, subject to satisfactory pe rformance of the agency and mutual agreement between ACTREC and the agency, at the sa me rates, terms and conditions of the original contract. Extension shall be subject to approval by the competent authority of ACTREC. .

Bidder financial standing: The bidder should not be under liquidation, court receivership or similar proceedings, should not be bankrupt. Bidder to upload undertaking to this effect with bid.

Bidder's offer is liable to be rejected if they don't upload any of the certificates / documents sought in the Bid document, ATC and Corrigendum if any.

• 5.Past Project Experience Proof for Past Experience and Project Experience clause: For fulfilling the experience criteria any one of the following documents may be considered as valid proof for meeting the experience criteria:a. Contract copy along with Invoice(s) with self-certification by the bidder that service/supplies against the invoices have been executed.b. Execution certificate by client with contract value.c. Any other document in support of contract execution like Third Party Inspection release note, etc.Proof for Past Experience and Project Experience clause: For fulfilling the experience criteria any one of the following documents may be considered as valid proof for meeting the experience criteria:a.
